The Four (4) Requirements:

1. Size of the FemCap is the most important (to be determined by YOUR obstetrical history and pelvic exam)
2. Prescription from your Health Care Professional
3. Name and Address of Yourself and your Health Care Professional
4. Payment as soon as we receive your payment we will rush your order

Determine your Femcap size based on your obstetrical history.
22mm FemCap - If you have NEVER been pregnant. See description.
26mm FemCap - If you have ever miscarried, had a termination even for 2 weeks of pregnancy or delivered via Caesarian-section.
30mm FemCap - If you have delivered a full-term baby vaginally.
The sizing is very important, so please consider your history very carefully.

Refund Policy: If for any reason your health care professional decides the FemCap is not suitable for you, return the unopened, tamper-proof-sealed package to FemCap for a full refund.

1. Contact a FemCap Provider near you, find one by selecting your state and city below.

2. Contact your existing health care provider and have him or her fax a prescription of your FemCap size to FemCap Inc.
....Fax number (858) 792-2624. If you do not have a fax, you can scan and email the prescription to contact@femcap.com.

3. Contact a Naturopathic doctor in your area.

4. Contact a Planned Parenthood in your area.

5. Contact the clinic nearest you who is part of the Family Planning Cooperative Purchasing Program.
